The CSMD team will observe the Veteran's Day holiday on November 11, 2024. We will respond to your phone calls, voicemails, and email messages beginning November 12, 2024 during normal business hours (8:00 a.m. - 4:30 p.m. CST).
As of October 8, 2024, the Practitioner vs. Peers report will be temporarily unavailable while technical updates are being made. We apologize for the inconvenience and will update this message when the report is returned to the user menu.
The CSMD Team is working diligently to provide the best customer service possible, but the best way for that to occur is through partnership. If you have an issue, you can send a screenshot of the errors or messages to CSMD.Admin@tn.gov. Please remember to include a contact phone number and as many details as possible.
TAKE NOTE: Users that click the “Forgot/Reset Password?” link will receive communications from the email address “CSMD.admin@tn.gov”. To prevent this email from being sent to your Junk/Spam folder, please add it as a “safe sender”. It is critical that the email associated with the account is up to date (must be unique, private and confidential) and that security question answers are appropriate in the account. These are critical elements of resetting the password. Please communicate to other members of your team and encourage them to log into their CSMD account and keep their information up to date by clicking “My Account”. If you have any questions, contact the CSMD Administrator.
Attention: Prescribers may notice receipt of emails from the CSMD that identify potential high-risk patients associated with their current practice. These will be displayed on your Clinical Notification panel on your home page.
To view Clinical Notifications associated with your account, click “View all” from the Clinical Notification panel on your account Home Page. This is a historical view and will always display the last five weeks of notifications. Please note the columns are sortable to assist in reviewing.
Remember: Upon your initial login to the CSMD, click on “MY ACCOUNT” (which is your profile) in the upper right corner to review and update all necessary information. You may click on the “Change Password” and “Security Questions” options on the left navigation bar and view/modify as needed.
When registering in the CSMD, the information that you provide must match the information as it appears on your Health Professional Board License. If you experience issues logging into the system (EX: invalid Board License, invalid DEA), please contact the CSMD Administrator via email at CSMD.Admin@tn.gov or by phone at 615-253-1305.
